Oculoplasty is a subspecialty that deals with diseases of the orbit eyelids, tear
ducts, and surrounding facial structures. There are various conditions that
affect these structures and they need specialised care and treatment.
Eyelid Conditions
One of the common eye lid problem is infection along the lid margin often associated with unhealthy oil secretion by the glands situated along the lid margin. Treatment involves lid hygiene, warm compresses and topical medications.
The other common eye lid problems include drooping of the upper eye lid, called Ptosis. It can be present since birth or develop later in life due to muscular or neurological problems. It can sometimes arise due to the presence of a tumor in the upper lid or aging.
The upper or lower lid can be turned inwards ( Entropion ) or outwards (Ectropion) and can result in long term redness, irritation and watering.
All the above conditions require a complete evaluation by a specialist and most often the treatment is surgical correction.
Watering of Eyes (Epiphora)
Often seen in infants and it is due to the passage for the tear outflow, the nasolacrimal duct, is blocked. There may be intermittent or constant wet eyes and when the block is significant there may actual rolling out of tears. Frequent infection is a possibility and will require antibiotics.
Commonly seen in elderly as well and it may be due to blocked duct or poor drainage of tears from age related lower lid laxity. Improper lid position can also lead to chronic watering.
Treatment of congenital duct obstruction depends upon the age of the child . In children upto 1 year of age simple massage will recanalize the passage and the symptoms are relieved. In older children minor surgery by passing a fine metal wire to open the duct ( Probing) or surgery ( DCR) may be required.
In adults the treatment depends upon the cause and often requires surgery.
Eyelid tumours
It can be a simple benign condition arising from blockage of oil secreting glands (chalazion ), infection of hair follicle called (Stye) or it could be a more serious condition like a malignant tumour.
Treatment involves proper diagnosis followed by medical or surgical depending upon the condition.
Thyroid Eye Disease
People with thyroid related problems can have eye diseases as well.
Often the symptoms include redness, irritation and occasionally pain. In long standing thyroid disease the eye may become more prominent as if it is bulging out of the socket ( Exophthalmos)
Medical management is the main stay of treatment and sometimes surgery may be required.
